Free Flu Clinic for NA Students Dec 10 NAMS

Flu Vaccines are required for all K-12 students by December 31, 2020 by Mass DPH.  

• Please print and complete 2 forms:  permission form and insurance form.  

• Sign up for a date and time for the vaccination

Public Health Nurse will be on site to vaccinate North Attleboro staff and residents (other than students)
