Health Benefits

 

North Attleborough offers the following health insurance plans.

HMO BLUE

BLUE CARE ELECT (PPO)  

 Employees pay between 10-25% of the monthly premium
 (depending on choice of plan) through weekly payroll deductions.

All plans are administered by Blue Cross/Blue Shield of Massachusetts.

(Employees may enroll upon hire or during the annual open enrollment)

Flexible Spending Account

 Employees have the ability to set up benefit deductions on a pre-tax basis.  This allows deductions to be “free” of taxes.  Flexible Spending Accounts (FSA) can also be established on a pre-tax basis which allows employees to set aside $ for:  

dental/vision care

medical deductibles

childcare expenses

long term care expenses

(Employees may enroll upon hire or during the annual open enrollment)

 

Basic Life Insurance

New employees may apply for $5000 of basic life insurance and $5000 of accidental death and dismemberment insurance for $1.06 per month.  

(This option is only available to new employees upon hire.)

Optional Life Insurance Plans

 New employees may apply for additional life insurance coverage up to 1x annual salary to a maximum of $74,000.  The cost is based on coverage amount selected.  

The current rate is .95 per $1000 of coverage.

 (This option is only available to new employees upon hire.)

Teachers Retirement Plan

 As a member of the Massachusetts Teachers Retirement System employees are eligible for retirement benefits through the Massachusetts Teachers Retirement Board (MTRB). This includes retirement allowance, health and life insurance.

Town Retirement Plan

As a member of the Massachusetts Public Employment System employees are eligible for retirement benefits through Massachusetts General Law-Chapter 32, which establishes benefits, contribution requirements, and an accounting and funds structure for all systems.

  Deferred Compensation Plans

Employees may supplement retirement by enrolling in any of the annuity plans (403b and 457 Plans) offered through the North Attleborough Public Schools and General Government. All employees may access 457 accounts, however only employees who participate in teacher's retirement may additionally enroll in 403b accounts.  Enrollment is these plans must be done through a financial investment agent.

403B and 457 Plan Information 

Employees may supplement retirement by enrolling in any of the annuity plans (403b and 457 Plans) offered through the North Attleborough Public Schools and General Government. All employees may access 457 accounts, however only employees who participate in teacher's retirement may additionally enroll in 403b accounts.  Enrollment is these plans must be done through a financial investment agent.

WORKER'S COMPENSATION  

Employee’s Role

Employee informs supervisor of accident/injury and if possible helps complete Form 101. 

If accident/injury requires medical treatment, employees are notified that the insurer has arranged for such attention at Sturdy Memorial Hospital.  Employees should state that their injury is work related and should direct all medical bills to the Town of North Attleborough Human Resources Dept., Woodcock Admin. Bldg.,6 Morse Street, North Attleborough, MA  02760 

All further medical treatment and/or time away from work should be communicated with the supervisor and Human Resources.

 Supervisor’s Role

Employee informs supervisor of accident/injury. 

Supervisor helps employee complete and sign the Medical Only Report of Injury AND Form 101.  Forms must be signed by supervisor and completed within 24 hours of accident or injury per Department of Industrial Accidents regulations. 

Supervisor should forward to Human Resources the completed and signed Medical Only Report of Injury and Form 101.

All work status and medical forms must be forwarded to Human Resources upon receipt.

         Human Resources Role

Submits Medical Only Report of Injury or Form 101 to MEGA Insurance Company.

Follows up with MEGA regarding claims and/or compensation due. Submits all claims/work status reports and medical notes to MEGA.  Keeps all copies in a separate employee medical file.

Communicates with employee, supervisor and payroll departments regarding out of work status and pay procedures.    

MEGA  Insurance Role 

Reviews claim and accepts or denies.  Denial letter is sent to employee/copy sent to Human Resources.

 If MEGA accepts claim they begin paying medical claims to doctor/hospital.  If over 5 days out of work, will start issuing checks to employee.  (The initial compensation checks will be sent to Human Resources to be forwarded to employee). 

MEGA will pay for 5 day waiting period if out of work over 21 days.  An adjustment to paid sick time will be made at that time.
