DOCUMENTS TO BRING TO REGISTRATION
You must bring the following documents with you in order to register:
your child's original birth certificate (we'll make a copy)
a copy of immunization records (including date of lead test)
a recent physical
proof of residence (i.e. utility or mortgage bill) Note: a driver's license is NOT proof of residence.
