Student Agreement

Every student, regardless of his or her age, must read and sign below:

I have read, understand and agree to abide by the terms of the Internet Safety/Technology Use Policy.  If I misuse my access to the North Attleborough School District’s technology equipment, networks, web site(s), and Internet access, I understand and agree that the privilege of the use of the School District’s technology equipment, networks, web site(s), and Internet access may be revoked, and school disciplinary action and/or legal action may be taken against me.

If I am under the age of eighteen, I have reviewed the Internet Safety Technology Use Policy with my parent/guardian and with the appropriate site administrator or supervising teacher, and they have explained the Internet Safety/Technology Use policy to me and answered all questions I had about the policy.

Parent/Guardian Agreement or Supplemental Agreement
for Students Who are Eighteen Years of Age, or Older

To be read and signed by a parent/guardian of a student who is under 18 years of age, or by a student who is eighteen years of age or older.

As the parent/guardian of the above-referenced student, I have read, understand and agree that my child shall comply with the terms of the North Attleborough School District’s Internet Safety/Technology Use Policy.  I understand that access is being provided to the students for educational purposes only, which may include post-secondary school or vocational purposes.  Although the School employs methods to filter or block offensive and controversial materials, I understand that it is impossible for the School to restrict access to all such materials and that it is the student's responsibility to abide by the Policy.

I hereby warrant and guarantee to the North Attleborough School District that I have carefully discussed and explained the Policy to the above-referenced student, and that I have either answered any and all questions the student had about the Policy, or personally relayed those questions to the appropriate site administrator or supervising teacher, or to the District’s Technology Coordinator.

I understand and acknowledge that Internet technology is a rapidly and constantly changing field.  Because of this continuous flux of technology, the School District may make changes in this policy and in the School District’s Contract for Internet/Technology Use at any time, with or without prior notice to students or other users, or parents.

I further understand and acknowledge that, it is impossible to guarantee that students will not accidentally or purposely access inappropriate material on the Internet, regardless of the efforts of the School District to prevent such access.

I further understand and agree that the above-referenced student may, from time to time, also be permitted access to web sites generated by the School District, or its affiliates, for the purpose of educational growth related to academics, extracurricular activities and/or vocational development.  I hereby expressly consent to the above-referenced student’s access, and I hereby understand and acknowledge that the School District’s Internet Safety/Technology Use Policy, and this Contract, applies equally to all such web sites and activities.

Additionally, I specifically understand and acknowledge that the above-referenced student’s out-of-school pursuit of any activities, contacts or opportunities located or developed through any such web site is not the responsibility of the School District and that the School District makes no claim or warranty as to the safety, harmlessness or appropriateness of any such activities, contacts or opportunities.

I hereby indemnify and hold harmless the North Attleborough School District, its Internet Service Provider (ISP), and all of the School District’s and ISP’s officers, employees, agents, servants, representatives, administrators, teachers, volunteers and staff from any and all losses, injuries, costs, fees, claims or damages resulting from the user's access to its computer network and the Internet, including, but not limited to, any fees or charges incurred through purchases of goods or services by the user.

In signing this agreement I give permission for my child to use the network/Internet connection and have witnessed my child's signature signifying his or her understanding as well.

Students will not have Internet access unless a signed Contract is returned.
